About EUTF Payments

The Hawaii Employer-Union Health Benefits Trust Fund or more commonly known as the EUTF provides medical, chiropractic, prescription drug, dental, vision, and life insurance benefits to all eligible State of Hawaii, City and County of Honolulu, County of Hawaii, County of Maui and County of Kauai employees and retirees.

The EUTF covers approximately 70,000 active employees plus 60,000 of their dependents, and 45,000 retirees plus 20,000 of their dependents.

Your EUTF ID Number begins with "HB" followed by seven (7) digits (HB_ _ _ _ _ _ _ ).

The number is located on your Notice for Health Insurance Premium Shortage or retiree coupon, or your CVS Caremark card. If you are unable to find your EUTF ID Number, please call EUTF at 808-586-7390.

You will find the amount on the correspondence that you received from the EUTF (i.e. Notice for Health Insurance Premium Shortage, retiree coupon). If you are not sure of the amount that you need to pay, please call EUTF at 808-586-7390.

For active employees, the pay period will be on the Notice for Health Insurance Premium Shortage that you received from the EUTF. Health benefit premiums are deducted from active employee paychecks after the pay period ends. For example, premiums for the July 1-15 pay period will be deducted on your July 15th paycheck for County employees and July 20th for State employees.

For retired employees, premiums are generally collected on a monthly basis. Retiree coupons are mailed to the retirees twice a year (June and December) and the month that you are paying for is located on the coupon. It is also located on the Notice for Health Insurance Premium Shortage.

If you received a Notice for Health Insurance Premium Shortage, payments are due by the 13th of the month. If payment is not received by the 13th, your health benefit plans will be cancelled.

All other payments are due by the 1st of the month.

Service fees apply to each EUTF payment processed online. The fee will be displayed before you confirm your payment.

You will receive a confirmation email after you make payment.

The routing number is the nine-digit series of numbers located at the bottom left area of the check.

The location of the account number may vary depending on your bank and may not always be located as shown in the below sample check. Please verify your account number with your bank prior to making payment on this site.

For First Hawaiian Bank checks, the account number is the third series of numbers on the bottom of the check.
